New Service at the Philadelphia Senior Center Philadelphia Senior Center is pleased to announce a new service available to seniors and their families who live in the Philadelphia area.

The Geriatric Assessment Program at the Philadelphia Senior Center is a joint program with Jefferson Family Medicine.

Participants will meet with a physician and a social service person for an evaluation of their physical, emotional and cognitive health. The senior will be given a list of recommendations that the team has made to share with their personal physician. The purpose of the program is to educate seniors and their families about services and programs that can help them continue to be active and independent in their communities. You do not have to receive medical care from Jefferson to participate in this program. Students who are training to care for older patients may participate in your education. About Jefferson Family Medicine Associates (JFMA)

Jefferson Family Medicine Associates (JFMA) is a teaching family medicine practice affiliated with Jefferson Medical College and Thomas Jefferson University Hospital. Their goals are to care for patients, teach new doctors and nurses, and learn ways to improve health care.
